Campus Personnel/Security Form
Please complete the Excel document providing names, email address and phone numbers of everyone you would like to have access to the Eyes On Alert mobile app. Please make sure to designate your personnel from your security staff by entering their name in the corresponding column. Specify those individuals you would like to have the ability to clear an emergency by placing a “Y” in the “Clear Emergency” column. Although anyone who has the mobile app can initiate an emergency, only those granted access can clear an emergency.

Eyes On Alert For Schools
School Security First Personnel Form
Please complete the Excel document providing names, email address and phone numbers of all personnel you would like to have access to the School Security First App. Specify those individuals you would like to have the ability to clear an emergency by placing a “Y” in the “Clear Emergency” column. Although anyone who has the mobile app can initiate an emergency, only those granted access can clear an emergency.
School Security First Parent & Student Form
Please complete the Excel document providing names, email address and phone numbers of all parents and students you would like to have access to the School Security First App. Please put the student’s name in the same column as the designated parent. If you have more than one parent, place the other parent’s information directly below the first parent and in the “Student’s Name” column place an “X” to designate the student belongs to both parents. See example below
